Echocardiographic identification of right-sided cardiac intracavitary thromboembolus in massive pulmonary embolism.

چکیده
In a patient with clinical features of massive pulmonary embolism, two-dimensional echocardiography demonstrated an echo-dense mass moving freely across the tricuspid valve between the right atrium and right ventricle. This mass was also visualized at angiography, which in addition confirmed extensive pulmonary embolism. At autopsy, the mass was found to be a coiled thrombus, one end of which had become wedged in a probe-patent foramen ovale. In another patient, bedside echocardiographic demonstration of thrombus in the right ventricular outflow tract was followed by immediate and successful embolectomy.

Metastatic right ventricular mass with intracavitary obliteration
Metastatic cardiac tumors are more common than the primary cardiac tumors. Cervical cancer metastasizing outside of the pelvis is commonly spread to the lungs, liver, bones and lymph nodes than to the heart. Right-sided metastasis to the heart is more common than to the left side. Intramural spread is more common than intracavitary growth of metastatic cardiac tumors leading to delayed clinical...
متن کاملSuccessful thrombolytic treatment of right sided massive intracardiac thrombus and pulmonary embolism.
A 67-year-old male patient was admitted to the emergency room with progressive dyspnoea. Blood gas analysis revealed hypoxia and hypocarbia. D-dimer was high. He was haemodynamically stable. Transthoracic echocardiography (TTE) showed massive right ventricular and mobile atrial thrombi and also right sided enlargement and flattening of the interventricular septum. Systolic pulmonary artery pres...
متن کاملRole of paraclinical assessment in management of massive pulmonary embolism: A case report
Despite all the diagnosis and treatment processes of pulmonary thromboembolism (PTE), it is still associated with a high rate of mortality. We describe a massive PTE case of a 73-year-old woman with unusual clinical manifestations.

متن کاملA Rare Silent Killer: Right Atrial Metastasis of Thyroid Hürthle Cell Carcinoma
Hürthle cell carcinoma (HCC) is a variant of a follicular carcinoma with a tendency to higher frequency of metastases and a lower survival rate. However, intracavitary cardiac metastases from thyroid HCC are extremely rare. We describe the case of a 57-year-old female with thyroid HCC, 5 years after total thyroidectomy, who presented with dyspnea associated with hypoxia and hypotension. The com...
